Community Supportive Housing (CSH)
Rapid Re-Housing: Rapid Re-Housing (RRH) is a program in our portfolio of projects designed to move households that have experienced homelessness to permanent housing quickly. The referrals come from the Office of Homeless Services (OHS) and are targeted at young families whose Head Of Household (HOH) is between 18-24 years of age. Supportive Case Management services are provided for one year. At the end of the year, HOH can obtain their own housing.
Chestnut Manor Supportive Housing: Chestnut Manor (CM) is for a single Head of Household (HOH) with a disability. The referrals come from the Office of Homeless Services (OHS). There is no predetermined length of stay for a CM Supportive Housing participant. Case Management supportive services are provided during the stay in the program. The goal is to assist with the identification of permanent housing. CM Supportive Housing Units are located in West Philadelphia close to the FFP office.
RSVP Supportive Housing: RSVP Supportive Housing targets families whose Head of Household (HOH) has a disability. The referrals come from the Office of Homeless Services (OHS). There is no predetermined length of stay for RSVP Supportive Housing families. Case Management supportive services are provided for the entire family during the program's stay, including educational planning and assistance with obtaining employment. The goal is to assist with the identification of permanent housing. RSVP Supportive Housing Units are located throughout the West Philadelphia area.
